Enter & View Report: Experiences of hospital discharge
Healthwatch Brent visited 4 care homes as part of a planned strategy to look at the experience of residents in the care home, with a particular focus on issues surrounding hospital discharge.
The visits aimed to obtain a better idea of the quality of care provided. This included information about the quality of care provided within the home itself, as well as evidence of the issues faced after hospital discharge for people with paid or unpaid carers, or who live in a care home.
